Qstation is a self-service web site where visitors can set up
(or arrange to have set up) simple or complex, single or multi-page,
database supported, web based structured data gathering forms.
These forms can be used for surveys, applications, Experimental
Data, Clinical Trials, subscriptions, complaints/suggestions,
business relationship forms, requests for information, polls and
more.
Uploading
Your Own HTML Files using Form Administration
- Create your HTML form, making sure all your form fields
are named properly.
- Upload your HTML forms to Qstation. We will provide
an FTP directory exclusively for you.
- Using your web browser, load your HTML forms into
the Qstation engine. At this point your form now has application
support and is database enabled, meaning responses will be stored
in the database. Users creating simple one page forms can move
directly to step 6 to deploy their forms.
- Make edits, specify decision tree (if any),
URL you want to send your respondent after they filled out
the forms, error messages, etc.
- Preview your pages.
Note: All forms are protected by the web server until they
are deployed. This means that a user name and password is need
to access a form at this point in the process.
- Deploy your pages. We will provide you with a URL
of your pages that you can use to send your respondents to.
- After deployment, you can define and run reports
on your survey. You can view the reports online or download
the files to your local hard drive so you can manipulate them
in a spreadsheet.
- Payment
No payment is required to use this service until you wish
to view your data. To get access to your data, you will need
to contact us at qstation@atypica.com
or by phone at 781-275-1890 ext 25. Provide us with your user
name, forms that you wish to purchase, number of weeks the
forms will be active and the expected number of respondents.
An invoice will be sent to you via email, fax or standard
mail. (An automated payment system will be available shortly.
Form Generator
If you don't know HTML (Hypertext Markup Language), you can use
the Form Generator to create HTML files to be loaded into the
Qstation engine. This replaces Step 1and Step 2 in "Uploading
Your Own HTML files."
Polls
In Qstation, polls are one-question, multiple-choice, surveys.
It is designed for those who want to enhance their sites with
quick-hitting but ephemeral snapshot polls. You can copy the generated
HTML form and paste it onto your own web page. When your user
submit the form, he will be sent to a results page hosted on Qstation.
